To start the registration process:

• Please fill out all of the paperwork. THIS MUST BE COMPLETED PRIOR TO ARRIVAL at the office.

P.S. Ignore the title that says Returning Users) User Name:

•  Your username is your first and last name all lowercase with no spaces in between
•  Your password is the first initial of your first name (capital) and the first initial of your last name
•  (lowercase) and your birthdate (month day year)

Example:

•  John Smith 09/22/1969
•  Username: johnsmith
•  Password: Js09221969
•  It will then prompt you to reset your password to a new private password.

To register,  you must fill out all SIX of the following tabs on the left side of the screen:

1. Medications
2. Allergies
3. Past Medical History
4. Skin Disease History
5. Social History
6. Family History

Press “Save and Continue” upon completion of each tab which is located on the bottom of each page Press “Logout” when all information is completed For patients with multiple family members, please call the office for a unique username for each family member, if you are using the same email address for all of you. Once you have the unique usernames, follow the instructions above for each member of your family.
